BSA Activity Consent Form (Permission Slip)
This form is the BSA permission slip used for all Troop outings.  Please print, fill out, and submit this form the the Troop Scout Master, Assistant Scout Master, or Activities Chairperson for each outing your son attends.

BSA Annual Health and Medical Record
This form is required by BSA at the time a scout joins and annually thereafter.  The form provides important medical information to be used only in a medical emergency if a parent or guardian is not present.  When appropriate please submit this form to the Troop’s Activity Chairperson.

Uniform Inspection Sheet
This form is being provided as a supplement to the information found in your son’s Boy Scout Handbook.  It is intended to assist you in properly positioning the various patches on our son’s official Class A uniform.  If you need further assistance please see the Troop’s Scout Master or Assistant Scout Master.

Merit Badge Counselor Application
If you are willing to help support our effort in delivering a quality program please consider becoming a Merit Badge Counselor.  Your professional knowledge and personal interests can help our scouts through the Merit Badge Program.  A very small time commitment on your part can help add great value to our program.
